Bottom line

TheraNest is a mental health EHR and practice management system that covers scheduling, notes, client communication, and insurance billing in one place. Its clearest strength is the credentialing service, which handles payer enrollment for up to three payers per therapist on eligible annual plans with no session commission. The main limitations are that pricing is only available on request, Ensai note-drafting is a paid add-on, and payer enrollment can take 60 to 120 days. Solo and small group therapy practices that bill insurance and want help getting on payer panels should consider it first.

What is TheraNest?

TheraNest is an electronic health record and practice management platform from Ensora Health, built for mental health therapists who run solo or group practices. It brings intake, scheduling, clinical notes, billing, and client communication into one system, so therapists spend less time on administrative work and more time with clients. The platform focuses on the paperwork that surrounds each session, including documentation, claims, and payments.

Day to day, therapists build intake forms and progress notes with the Dynamic Form Builder and book sessions in a calendar that handles recurring appointments, group rosters, and room assignments. Automated text, email, and phone reminders go out before appointments, and no-show fees can be applied automatically. Clients complete intake forms, request appointments, message the practice securely, and pay balances from any browser through the client portal. Billing runs through automated invoice creation, claim submission, and ERA posting, with rules set once per payer so that exceptions appear in a single queue. Calendars sync with Google Calendar, Microsoft Outlook, and iCloud. Ensai Session Assistant, a paid add-on, drafts progress notes for review and produces AI case summaries before each session.

TheraNest serves solo therapists and group practices, including teams with multiple clinicians, interns, and supervisors, and it supports multi-location operations. Pricing is provided on request. Practices can start a 21-day free trial, and the product is sold on Advanced and Premier practice management plans. Credentialing support covering up to three payer enrollments per therapist comes with eligible annual plans. Optional Revenue Cycle Management is billed at a flat fee rather than a percentage of what the practice earns.

What sets TheraNest apart is its built-in credentialing service. Enrollment specialists prepare and submit payer applications, track their progress, and follow up with payers until each one makes a decision, and Ensora takes no commission on sessions. Most enrollments take about 60 to 120 days from start to finish, so practices planning to join insurance panels should begin early. Reporting can filter by staff, payer, or date range and export to Excel without an add-on, and the platform is HITRUST certified and HIPAA compliant.
